เราได้จัดงาน Healthcare Day โดยมี 3 บริษัทเข้าร่วม ได้แก่ BCH BDMS และ CHG ภาพรวมเป็นกลาง โดยประเด็นสำคัญได้แก่: 1) การอัพเดทการดำเนินงานหลังเหตุแผ่นดินไหว โดยทั้งสามบริษัทกล่าวว่าผลกระทบไม่มีนัยสำคัญ 2) โอกาสและความท้าทายในปี 2568 3) มุมมองเกี่ยวกับบริการผู้ป่วยสิทธิประกันสังคม (SC) เราคงมุมมองเชิงบวกต่อ BCH (OUTPERFORM, ราคาเป้าหมาย 20 บาท) โดยได้รับปัจจัยหนุนจากการฟื้นตัวของผลประกอบการ โดยคาดว่ากำไรปกติจะเติบโต 15% ในปี 2568 และ BDMS (OUTPERFORM, ราคาเป้าหมาย 32 บาท) เพราะคาดว่ากำไรปกติจะเติบโตดีที่ 8% YoY ในปี 2568 และอยู่ในตำแหน่งที่ดีกว่าคู่แข่งโดยตรงอย่าง BH
BCH: รายได้จากธุรกิจโรงพยาบาลของ BCH เติบโต ~3% YoY ใน 1Q68 โดยการเติบโตเพียงเล็กน้อยสะท้อนถึงรายได้จากบริการ IPD ที่ลดลงจากจำนวนผู้ป่วยชาวคูเวตที่ลดลง ในขณะที่รายได้จากบริการ OPD ยังเติบโต บริษัทได้รับผลกระทบเพียงเล็กน้อยจากเหตุแผ่นดินไหวเมื่อวันที่ 28 มี.ค. โดยกลับมาดำเนินการตามปกติหลังจากมั่นใจในความปลอดภัยของผู้ป่วย ผู้ป่วยจากเมียนมาคิดเป็น 3% ของรายได้ โดยหลักๆ อยู่ที่ โรงพยาบาลเกษมราษฎร์ ศรีบุรินทร์ ในเชียงราย BCH พบว่าผู้ป่วยมีการเลื่อนการเข้ารับการรักษาออกไปบ้าง แต่คาดว่าจะฟื้นตัวในระยะเวลาอันใกล้นี้ ในปี 2568 บริษัทตั้งเป้าการเติบโตของรายได้ที่ 10-15% โดยได้รับแรงหนุนจากการอัพเกรดโรงพยาบาล บริการใหม่ๆ จำนวนผู้ป่วยชาวต่างชาติที่เพิ่มขึ้น และการดำเนินงานที่ปรับตัวดีขึ้นที่โรงพยาบาลใหม่ 3 แห่ง BCH มองว่าความท้าทายคือภาวะเศรษฐกิจชะลอตัวและพฤติกรรมผู้ป่วยที่เปลี่ยนไป นอกจากนี้ BCH ยังได้ขยายธุรกิจสู่ตลาดใหม่ด้วยการลงนามข้อตกลงกับรัฐบาลมัลดีฟส์ในการรักษาผู้ป่วยชาวมัลดีฟส์ ซึ่งคาดว่าจะช่วยหนุนให้รายได้จากผู้ป่วยชาวต่างชาติปรับตัวเพิ่มขึ้นใน 2Q68 BCH คาดว่าสำนักงานประกันสังคมจะคงอัตราการจ่ายเงินค่ารักษาพยาบาลโรคที่มีค่าใช้จ่ายสูงไว้ที่ 12,000 บาท/AdjRW ในปี 2568 โดยได้รับผลกระทบจำกัดจากการตัด 3 โรคออกจากการเป็นโรคเร่งด่วน (น้อยกว่า 1% ของรายได้) และเกณฑ์คัดกรองที่เข้มงวดขึ้นสำหรับการรักษาโรคอ้วน (รายได้ไม่มีนัยสำคัญ)
BDMS: BDMS รายงานรายได้จากธุรกิจโรงพยาบาลเติบโต 6-7% YoY ในเดือนม.ค.-ก.พ. โดยได้รับแรงหนุนจากบริการผู้ป่วยชาวต่างชาติ (เติบโต 10-15% YoY) ขณะที่ผลกระทบจากเหตุแผ่นดินไหวเมื่อวันที่ 28 มี.ค. มีจำกัด ใน 1H68 บริษัทตั้งเป้าการเติบโตของรายได้ที่ 7-8% และ EBITDA margin ที่ 24-25% ในระยะ 2-3 ปีข้างหน้า บริษัทวางแผนเพิ่มสัดส่วนรายได้จากบริการผู้ป่วยชาวต่างชาติเป็น 30% จาก 28% ในปี 2567 โดยจะมากจากการเติบโตของผู้ป่วยจากตะวันออกกลางและ CLMV รวมถึงตลาดใหม่ๆ ที่มีศักยภาพ เช่น ซาอุดีอาระเบีย และอิสราเอล บริษัทพบว่ามีผลกระทบจำกัดจากเงื่อนไข copayment สำหรับกรมธรรม์ประกันสุขภาพฉบับใหม่ และตั้งเป้าเพิ่มรายได้จากผู้ป่วยชาวไทยที่มีประกันสุขภาพภาคเอกชน เพื่อให้เกิดการใช้ประโยชน์จากสินทรัพย์ให้ดีขึ้น และกระจายการดำเนินงาน สิ่งที่บริษัทกังวล คือ การแข่งขันที่สูงขึ้น
CHG: CHG มีแนวโน้มที่จะรายงานรายได้เติบโตเล็กน้อย YoY ใน 1Q68 ขณะที่ได้รับผลกระทบจากเหตุแผ่นดินไหวเพียงเล็กน้อย บริษัทตั้งเป้าการเติบโตของรายได้อย่างน้อย 10% ในปี 2568 โดยเน้นที่บริการ SC และเพิ่มรายได้ต่อหัวผ่านการให้บริการทางการแพทย์ที่ซับซ้อนมากขึ้น CHG คาดว่าสำนักงานประกันสังคมจะคงอัตราการจ่ายเงินค่ารักษาพยาบาลโรคที่มีค่าใช้จ่ายสูงไว้ที่ 12,000 บาท/AdjRW ในปี 2568 โดยได้รับผลกระทบจำกัดจากการตัด 3 โรคออกจากการเป็นโรคเร่งด่วน (น้อยกว่า 1% ของรายได้) แต่จะได้รับผลกระทบอยู่บ้างจากเกณฑ์คัดกรองที่เข้มงวดขึ้นสำหรับการรักษาโรคอ้วน (1-2% ของรายได้) ทั้งนี้แม้ว่า CHG จะมีมุมมองที่เป็นบวกมากขึ้นเกี่ยวกับการจ่ายค่ารักษาพยาบาลโรคที่มีค่าใช้จ่ายสูง (AdjRW>2) แต่บริษัทยังคงระมัดระวังเกี่ยวกับความไม่แน่นอน โดยจะมุ่งเน้นการบริหารจัดการต้นทุนเพื่อลดผลกระทบที่อาจเกิดขึ้น
ความเสี่ยง คือ เหตุการณ์ที่ไม่คาดคิด เศรษฐกิจโลกชะลอตัวลง และความเสี่ยงทางภูมิรัฐศาสตร์ซึ่งจะส่งผลกระทบทำให้ผู้ป่วยชะลอการเข้าใช้บริการ การดำเนินงานที่อ่อนแอของโรงพยาบาลที่เปิดใหม่ การแข่งขันรุนแรง การขาดแคลนบุคลากร และความเสี่ยงด้านกฎหมาย สำหรับกลุ่มการแพทย์ เรามองว่าปัจจัยเสี่ยงด้าน ESG ที่สำคัญ คือ ความปลอดภัยของผู้ป่วย (S) ซึ่งบริษัทต่างๆ นำเอาระบบบริหารคุณภาพต่างๆ มาใช้สำหรับกระบวนการดูแลผู้ป่วยอย่างต่อเนื่อง
Event: Healthcare Day
We hosted a Healthcare Day event with three companies participating: BCH, BDMS and CHG. The tone overall tone neutral. Key points include: 1) operational update after the earthquake, with all three saying impact was insignificant, 2) opportunities and challenges in 2025, 3) view on SC services. We maintain our positive view on BCH (Outperform, TP of Bt20/share) backed by an earnings turnaround with 15% core earnings growth in 2025 and BDMS (Outperform, TP of Bt32/share) on healthy core earnings growth of 8% in 2025 and better positioning than direct peer BH.
BCH (Lead pick): BCH’s hospital revenue grew ~3% YoY in 1Q25, with the small growth reflecting lower IPD revenue on fewer Kuwaiti patients, while OPD revenue continues to grow. It has had insignificant impact from the March 28 earthquake, resuming normal operations after ensuring patient safety. Patients from Myanmar account for 3% of its revenue, mainly at Kasemrad Hospital Sriburin in Chiang Rai. BCH has noticed some postponements but expects to recover in the near-term. In 2025, it targets 10-15% revenue growth driven by hospital upgrades, new services, international patient growth and improved operations at three new hospitals, while addressing challenges lying in an economic slowdown and changing patient behavior. BCH has also entered a new market by signing an agreement with the Maldives government to treat Maldivian patients, which is expected to boost revenue from international patients in 2Q25. It expects the Social Security Office to maintain the payment rate for high-cost care at Bt12,000/AdjRW in 2025, with limited impact from the removal of three diseases from the urgent disease program (less than 1% of revenue) and stricter screening criteria for morbid obesity treatment (insignificant revenue).
BDMS (Top pick): BDMS reported a 6-7% YoY growth in hospital revenue in January-February, driven by international patient services (a 10-15% YoY growth), with minimal impact from the March 28 earthquake. It targets 7-8% revenue growth and a 24-25% EBITDA margin in 1H25. Over the next few years, it plans to grow revenue contribution from international patients to 30% from 28% in 2024, growing patients from the Middle East and CLMV and new potential markets including Saudi Arabia and Israel. It sees limited impact from the copayment requirement for new health insurance policies and aims to increase revenue from Thai patients with private health insurance, improving asset utilization and operational diversification. Its concern is rising competition.
CHG (Neutral): CHG is likely to report a small YoY growth in hospital revenue in 1Q25, with minimal impact from the earthquake. The company targets at least 10% revenue growth in 2025, focusing on SC services and increasing revenue per patient through more complex medical specialties. It expects the Social Security Office to maintain the high-cost care payment rate at Bt12,000/AdjRW in 2025, with limited impact from the removal of three diseases from the urgent disease program (less than 1% of revenue) but some impact from stricter screening for morbid obesity treatment (1-2% of revenue). Although CHG has a more positive outlook on payment for high-cost care (AdjRW>2), it remains cautious about the uncertainties. It will focus on managing costs to mitigate any possible impact.
Risks include unpredictable events, a global economic slowdown and geopolitical risk that would interrupt patient traffic, slow ramping up of new facilities, intense competition, workforce shortage and regulatory risk. For the healthcare service sector, we see ESG risk as patient safety (S) that companies have adopted a variety of quality assurance systems to provide continuous patient care.
