BCH – 1Q66: กำไรต่ำกว่าคาดเพราะต้นทุนสูงขึ้น

BCH รายงานกำไรสุทธิ 1Q66 ที่อ่อนแอจำนวน 254 ลบ. ลดลง 87% YoY จากฐานสูงของบริการโควิด-19 ใน 1H65 และลดลง 6% QoQ กำไรออกมาต่ำกว่า consensus คาด 9% โดยมีสาเหตุมาจากรายได้และ EBITDA margin ที่อ่อนแอลงเพราะต้นทุนสูงขึ้น ซึ่งหนุนให้เราปรับประมาณการกำไรลดลง ทั้งนี้แม้ valuation ดูถูกหลังจากราคาหุ้นปรับตัวลดลงมาแล้ว 15% จากจุดสูงสุดล่าสุด แต่เรายังคงคำแนะนำ tactical call ระยะ 3 เดือนสำหรับ BCH ไว้ที่ NEUTRAL เนื่องจากเรามองว่าในระยะสั้นหุ้น BCH ยังคงขาดปัจจัยบวกเรื่องความเชื่อมั่นต่อผลประกอบการจนกว่ากำไรจะกลับคืนสู่ทิศทางขาขึ้นใน 2H66 ราคาเป้าหมายสิ้นปี 2566 ที่เราปรับใหม่อยู่ที่ 22 บาท/หุ้น (ลดลงจาก 24 บาท/หุ้น)

1Q66: กำไรต่ำกว่าคาด BCH รายงานกำไรสุทธิ 1Q66 ที่อ่อนแอจำนวน 254 ลบ. ลดลง 87% YoY จากฐานสูงของบริการโควิด-19 ใน 1H65 และลดลง 6% QoQ หากไม่รวมรายการพิเศษเกี่ยวกับบริการโควิด-19 ใน 4Q65 พบว่ากำไรปกติลดลง 34% QoQ กำไรออกมาต่ำกว่า consensus คาด 9% โดยมีสาเหตุมาจากรายได้และ EBITDA margin ที่อ่อนแอลงเพราะต้นทุนสูงขึ้น

รายการที่สำคัญ:

  • รายได้จากกิจการโรงพยาบาลอยู่ที่ 2.7 พันลบ. ใน 1Q66 ลดลงแรง 62% YoY โดยมีสาเหตุหลักมาจากรายได้จากบริการโควิด-19 ที่ลดลง และลดลง 4% QoQ จากปัจจัยฤดูกาล
  • เรามองว่าสิ่งที่อ่อนแอคือ EBITDA margin ที่ 5% ใน 1Q66 (ลดลงจาก 23.4% ใน 4Q65) ทั้งนี้แม้รายได้ลดลง QoQ แต่ต้นทุนกิจการโรงพยาบาลของ BCH เพิ่มขึ้น 3% QoQ เนื่องจากค่าใช้จ่ายบุคลากรเพิ่มขึ้นและผลการดำเนินงานของโรงพยาบาลใหม่อ่อนแอลงจากบริการ COVID-19 ที่ลดลง

ปรับประมาณการกำไรลดลง เราปรับประมาณการกำไรปกติของ BCH ลดลง 24% ในปี 2566 และ 15% ในปี 2567 เพื่อสะท้อนผลประกอบการ 1Q66 ที่แย่กว่าคาด และค่าใช้จ่ายในการดำเนินงานที่สูงขึ้น ซึ่งไปหักล้างกำไรส่วนเพิ่มจากการปรับอัตราค่าบริการเหมาจ่ายรายหัวผู้ประกันตนในโครงการประกันสังคมเพิ่มขึ้น 10% (มีผลบังคับใช้ในเดือนพ.ค.) ทั้งนี้ หลังจากปรับประมาณการ เราคาดการณ์กำไรปกติของ BCH ที่ 1.4 พันลบ. ในปี 2566 (-66% YoY) และ 1.7 พันลบ. ในปี 2567 (+20% YoY) เราคาดว่ากำไรปกติ 2Q66 ของ BCH จะลดลง YoY แต่จะอยู่ในระดับทรงตัว QoQ เนื่องจากผลกระทบจากช่วงโลว์ซีซั่นจะได้รับการชดเชยจากการปรับอัตราค่าบริการเหมาจ่ายรายหัวผู้ประกันตนในโครงการประกันสังคมเพิ่มขึ้น เราคาดว่ากำไรปกติของ BCH จะปรับตัวดีขึ้นใน 2H66 (+HoH) และมีแนวโน้มปรับตัวเพิ่มขึ้น โดยจะเริ่มเห็นการเติบโต YoY ใน 4Q66 ด้วยแรงหนุนบริการที่ไม่เกี่ยวกับโควิด-19 ที่เติบโตเพิ่มขึ้นและการดำเนินงานที่ปรับตัวดีขึ้นที่โรงพยาบาลใหม่ 3 แห่ง (อรัญประเทศ ปราจีนบุรี และสปป. ลาว)

ปรับราคาเป้าหมายลดลง ราคาเป้าหมายสิ้นปี 2566 อ้างอิงวิธี DCF ที่เราปรับใหม่อยู่ที่ 22 บาท/หุ้น (ลดลงจาก 24 บาท/หุ้น) โดยอิงกับ WACC ที่ 6.2% และอัตราการเติบโตระยะยาวที่ 3% เรายังคงคำแนะนำ NEUTRAL เนื่องจากเรามองว่าในระยะสั้นหุ้น BCH ยังคงขาดปัจจัยบวกเรื่องความเชื่อมั่นต่อผลประกอบการ ปัจจุบันหุ้น BCH เทรดที่ PE ปี 2566 ระดับ 34 เท่า หรือระดับใกล้เคียง -1SD ของ PE เฉลี่ยในอดีต (ปี 2558-2562) เรามองว่าโอกาสเข้าสะสมที่น่าสนใจคือระดับราคาหุ้นที่ต่ำกว่า 17.4 บาท/หุ้น เพราะสะท้อนระดับ PE ปี 2566 ที่ 32 เท่า หรือ -2SD ซึ่งน่าจะเป็นระดับที่จำกัด downside ของราคาได้

ปัจจัยเสี่ยง เหตุการณ์ที่ไม่คาดคิด เช่น การเกิดโรคระบาดใหญ่ที่จะส่งผลกระทบทำให้ผู้ป่วยชะลอการเข้าใช้บริการ การแข่งขันรุนแรง การขาดแคลนบุคลากร และความเสี่ยงด้านกฎหมาย

BCH reported a poor 1Q23 net profit of Bt254mn, down 87% YoY off the high base of COVID-19 services in 1H22 and 6% QoQ, missing consensus by 9% on lower revenue and EBITDA margin from rising cost. In response, we have cut our 2023F. Despite a discounted valuation after a fall of 15% in share price from a recent peak, we maintain our 3-month tactical call of Neutral as it lacks near-term earnings conviction until 2H23. Our new end-2023 DCF TP is Bt22/share (down from Bt24/share).

1Q23: A miss. BCH reported a weak 1Q23 net profit of Bt254mn, down 87% YoY off the high base from COVID-19 services in 1H22 and down 6% QoQ. Excluding extra items related to COVID-19 services in 4Q22, core profit fell 34% QoQ, 9% below consensus on lower revenue and EBITDA margin from rising cost.

Highlights:

  • Hospital revenue was Bt2.7bn in 1Q23, plummeting 62% YoY due to lower revenue from COVID-19 services and down 4% QoQ on seasonality.
  • We see EBITDA margin as a weak point at 22.5% in 1Q23 vs. 23.4% in 4Q22. Despite the QoQ drop in revenue, cost of hospital operations increased 3% QoQ on rising personnel expense and weaker performance of new hospitals from lower COVID-19 services.

Earnings cut. We cut core earnings by 24% in 2023 and 15% in 2024 to factor in the 1Q23 miss and rising operating cost that overshadows the earnings added by the 10% rise in SC general capitation (effective in May). After the revision, we expect core earnings at Bt1.4bn in 2023 (-66% YoY) and Bt1.7bn in 2024 (+20% YoY). We expect 2Q23 core earnings to drop YoY but be flat QoQ as the lower season will be offset by rising SC general capitation. We expect better core earnings in 2H23 (+HoH) and look for an earnings uptrend with YoY growth to start in 4Q23 on growing non-COVID-19 services and improving operations at three new hospitals (Aranyaprathet, Prachinburi and Lao PDR).

Lower TP. Our new end-2023 DCF TP is Bt22/share (down from Bt24/share) based on WACC at 6.2% and LT growth at 3%. We stay Neutral as we see it as lacking near-term earnings conviction. BCH is trading at 34x 2023PE, close to
-1SD of its historical average (2015-19). In our view, an opportunity to accumulate would be when share price falls to below Bt17.4/share as this implies 32x 2023PE or -2SD, suggesting downside support.

Risks. Unpredictable events such as another pandemic that would interrupt patient traffic, intense competition, staff shortages and regulatory risk.
