1Q26: Weak, as expected
BCH reported a 1Q26 net profit of Bt268mn, down 17% YoY but up 3% QoQ, in line with INVX and market expectations of a weak quarter. The decline was primarily from higher operating costs related to facility renovations and medical staff expenses amidst weak revenue. We expect 2Q26 to be a soft quarter as revenue is expected to continue low on cautious spending and the impact of the US-Iran war on the Middle East segment. Catalyst is the earnings recovery expected in 2H26, driven by strategies to increase revenue from international patients and SC services. OUTPERFORM with an end-2026 DCF-based TP of Bt12.
1Q26: Weak as expected. BCH reported a 1Q26 net profit of Bt268mn, down 17% YoY but up 3% QoQ, in line with INVX and market expectations for a weak quarter. The decline was from higher operating costs related to facility renovations and medical staff expenses amidst weak revenue.
1Q26 highlights:
• Revenue was Bt2.9bn, flat YoY but down 2% QoQ. Growth was seen in IPD services (29% of revenue, up 1% YoY but down 4% QoQ), driven by an increase in patients from the Middle East and Myanmar, and in social security services (SC, 36% of revenue, up 4% YoY and 6% QoQ) due to a higher number of insured persons. The weak point was revenue from OPD services (35% of revenue, down 5% YoY and 7% QoQ) reflecting the impact of the Thai-Cambodian border closure and a decline in seasonal flu patients nationwide.
• EBITDA margin softened to 22.1% from 24.7% in 1Q25 but was flat QoQ due to higher operating costs related to facility renovations and medical staff expenses amidst weak revenue.
Soft outlook in 2Q26. 1Q26 profit accounted for 21% of our full-year forecast and we leave it unchanged. In 2Q26, we expect core earnings to drop YoY as revenue is expected to continue low from cautious spending and the impact of the US-Iran war on the Middle East segment. However, the seasonal QoQ drop can turn to growth if the SSO pays additional SC revenue for the 26 chronic diseases service (historically paid in 2Q or 3Q). We expect better earnings in 2H26, boosted by new revenue drivers such as the expansion of services at Kasemrad Mae Sai Hospital in 2Q26 to attract Myanmar patients, penetrating new markets like Qatar, and focusing on SC services. BCH will collaborate with private corporations to provide services such as annual health check-ups and vaccinations. Our end-2026 DCF-based target price is Bt12, based on a WACC of 7.5% and LT growth of 2.5%.
Risks. Change in SC reimbursement, slower patient traffic and cost burden at new facilities. We see ESG risk as patient safety (S): BCH has adopted a variety of quality assurance systems to provide continuous patient care.
