2Q26: In-line; special dividend announced

BCH reported a 2Q26 net profit of Bt343mn (-12% YoY but +28% QoQ), in line with our and market expectations. The YoY decline was driven by weaker revenue and a lower EBITDA margin, while QoQ growth was supported by stronger SC revenue. BCH announced a DPS of Bt0.35 (3.2% yield), including a special dividend of Bt0.20. We expect earnings momentum to improve in 2H26, followed by an earnings turnaround in 2027. Additional upside could come from a potential SC reimbursement rate hike, with greater clarity likely in 4Q26. BCH is now our top pick in the healthcare sector. OUTPERFORM with a mid-2027 TP of Bt13/share.

2Q26: In line results. BCH reported a 2Q26 net profit of Bt343mn, down 12% YoY but up 28% QoQ, in line with our and market expectations. The YoY earnings decline was driven by weaker revenue and a lower EBITDA margin, while the QoQ improvement was supported by stronger Social Security (SC) revenue, including an additional Bt71mn reimbursement for the treatment of 26 chronic diseases.

Key highlights;
• SC revenue offset weak self-pay demand. Revenue was Bt3.0bn, down 1% YoY but up 3% QoQ. SC service (39% of revenue) grew 4% YoY and 10% QoQ, driven by higher treatment intensity for complex diseases, referral-center service and the additional reimbursement for 26 chronic diseases. In contrast, self-pay revenue remained soft. OPD revenue (35% of revenue) fell 6% YoY but rose 3% QoQ, while IPD revenue (27% of revenue) declined 2% YoY and 6% QoQ. Management attributed the weakness to softer revenue from aesthetic centers and lower Cambodian patient volumes amid the Thailand-Cambodia border conflict since June 2025. Nevertheless, revenue from Middle Eastern patients continued to grow, with a noticeable shift from outpatient care toward inpatient treatment and more complex cases, supporting higher revenue intensity per patient.
• EBITDA margin was 25.0% in 2Q26, contracting 1.1ppts YoY as operating costs rise amid weak revenue growth. However, this contraction is narrower than the 2.7ppts YoY drop in 1Q26, reflecting effective cost control.

Special dividend announced. BCH announced a total DPS of Bt0.35 (3.2% dividend yield), comprising an interim dividend of Bt0.15 and a special dividend of Bt0.20. XD is on August 27, 2026 and payment date is September 11, 2026.

Earnings forecast maintained. 1H26 core profit accounted for 48% of our full-year forecast. We maintain our earnings projections and expect earnings momentum to strengthen in 2H26, supported by new facilities and rising healthcare demand from both Thai and international patients. Additional upside could come from a potential SC reimbursement rate hike, with greater clarity expected in 4Q26. Our sensitivity analysis suggests that a 5% increase in average reimbursement rates would lift our 2027 earnings forecast by 13%. We rollover our DCF valuation to mid-2027 and raise our TP to Bt13/share (from end-2026 TP at Bt12), based on a WACC of 7.5% and a long-term growth rate of 2.5%.

Risks. Change in SC reimbursement, slower patient traffic and cost burden at new facilities. We see ESG risk as patient safety (S): BCH has adopted a variety of quality assurance systems to provide continuous patient care.
