BH - 4Q67: ดีกว่าคาดจากค่าใช้จ่ายภาษีที่ลดลง

BH รายงานกำไรสุทธิ 4Q67 ที่ 1.9 พันลบ. เพิ่มขึ้น 11% YoY แต่ลดลง 3% QoQ ดีกว่าที่ INVX และตลาดคาดการณ์ไว้ 7% หลักๆ เกิดจากค่าใช้จ่ายภาษีที่ลดลง เราคงคำแนะนำ NEUTRAL สำหรับ BH เนื่องจากกำไรปกติปี 2568-2569 จะเติบโตในระดับต่ำ เพราะจะถูกฉุดรั้งโดย EBITDA margin ที่มีแนวโน้มปรับตัวลดลงจากรายได้จากการรักษาโรคซับซ้อน (revenue intensity) ที่กลับสู่ระดับปกติ และการเปิดโรงพยาบาลแห่งใหม่ในภูเก็ต ราคาเป้าหมายสิ้นปี 2568 อ้างอิงวิธี DCF ของเราอยู่ที่ 250 บาท/หุ้น (WACC ที่ 7.5% และการเติบโตระยะยาวที่ 3%)

4Q67: ดีกว่าคาดจากค่าใช้จ่ายภาษีที่ลดลง BH รายงานกำไรสุทธิ 4Q67 ที่ 1.9 พันลบ. เพิ่มขึ้น 11% YoY แต่ลดลง 3% QoQ ดีกว่าที่ INVX และตลาดคาดการณ์ไว้ 7% หลักๆ เกิดจากค่าใช้จ่ายภาษีที่ลดลง เมื่อหักกำไรจากอัตราแลกเปลี่ยนออก พบว่ากำไรปกติเติบโต 11% YoY แต่ลดลง 5% QoQ กำไรปกติที่เติบโต YoY ได้แรงหนุนจาก EBITDA margin ที่สูงขึ้น และค่าใช้จ่ายภาษีที่ลดลง ในขณะที่กำไรปกติที่ลดลง QoQ เป็นผลมาจากค่าใช้จ่าย SG&A ที่เพิ่มขึ้นจากปัจจัยฤดูกาล สำหรับปี 2567 BH รายงานกำไรสุทธิ 7.8 พันลบ. เพิ่มขึ้น 11% YoY BH ประกาศจ่ายเงินปันผล 3 บาท/หุ้น จากผลการดำเนินงานงวด 2H67 คิดเป็นอัตราผลตอบแทนจากเงินปันผล 1.6% XD วันที่ 19 มี.ค. และจ่ายเงินปันผลวันที่ 13 พ.ค.


รายการที่สำคัญ
• (-) บริการผู้ป่วยต่างชาติชะลอตัว รายได้ใน 4Q67 อยู่ที่ 6.5 พันลบ. ลดลง 1% YoY (แต่เพิ่มขึ้น 1% QoQ) เพราะได้รับแรงกดดันจากรายได้จากบริการผู้ป่วยต่างชาติ (64% ของรายได้) ที่ลดลง 6% YoY (และ 1% QoQ) และมีแนวโน้มปรับตัวลดลงต่อเนื่องมาจาก 3Q67 ซึ่งเป็นผลมาจากรายได้จากการรักษาโรคซับซ้อน (revenue intensity) ที่ลดลง อย่างไรก็ดี บริการผู้ป่วยชาวไทย (36% ของรายได้) เติบโตดี โดยรายได้เพิ่มขึ้น 10% YoY และ 4% QoQ

• (+) ค่าใช้จ่ายภาษีลดลง มาอยู่ที่ 279 ลบ. ลดลง 23% YoY และ 34% QoQ เนื่องจาก BH มีสิทธิประโยชน์ทางภาษีจากมาตรการส่งเสริมการลงทุนเพื่อพัฒนาชุมชนและสังคมโดยสำนักงานคณะกรรมการส่งเสริมการลงทุน (BOI)
ปรับประมาณการกำไรเล็กน้อย เราปรับประมาณการกำไรปกติปี 2568 และปี 2569 ของ BH เพิ่มขึ้นปีละ 2% ซึ่งหมายความว่ากำไรปกติจะเติบโต 3% ในปี 2568 การเติบโตของกำไรปกติในระดับต่ำอิงกับมุมมองที่ว่า EBITDA margin จะลดลงเล็กน้อยมาอยู่ที่ 38.9% (จาก 39.4% ในปี 2567) เนื่องจากรายได้จากการรักษาโรคซับซ้อน (revenue intensity) มีแนวโน้มปรับตัวกลับสู่ระดับปกติต่อเนื่องมาจาก 3Q67 สำหรับปี 2569 เราคาดว่ากำไรปกติของ BH จะสะดุดลงเล็กน้อย โดยมีแรงฉุดรั้งจากการเปิดโรงพยาบาลแห่งใหม่ในภูเก็ตใน 2H69 เราคงคำแนะนำ NEUTRAL สำหรับ BH โดยให้ราคาเป้าหมายสิ้นปี 2568 อ้างอิงวิธี DCF ที่ 250 บาท/หุ้น (WACC ที่ 7.5% และการเติบโตในระยะยาวที่ 3%)

4Q24: Beat on lower tax expense

BH reported a 4Q24 net profit of Bt1.9bn, up 11% YoY but down 3% QoQ, beating INVX and market estimates by 7% on lower tax expense. We maintain Neutral on BH based on its unexciting core earnings growth in 2025-26, dragged down a downtrend in EBITDA margin from normalizing revenue intensity and opening a new hospital in Phuket. Our end-2025 DCF TP is Bt250/share (WACC at 7.5% and long-term growth at 3%).


4Q24: Beat on lower tax expense. BH reported a 4Q24 net profit of Bt1.9bn, up 11% YoY but down 3% QoQ, beating INVX and market estimates by 7% on lower tax expense. Excluding FX gain, core profit grew 11% YoY but slipped 5% QoQ. The YoY growth was driven by higher EBITDA margin and lower tax expense while the QoQ drop was due to a seasonal rise in SG&A expense. Net profit in 2024 was Bt7.8bn, growth of 11%. BH announced a DPS of Bt3 on 2H24 operations, 1.6% dividend yield. XD is March 19 and payment is May 13.


Highlights:
• (-) Slow international patient services. Revenue was Bt6.5bn in 4Q24, down 1% YoY (but up 1% QoQ) dragged down by lower revenue from international patient services (64% of revenue), which fell 6% YoY (and 1% QoQ), continuing from 3Q24 on lower revenue intensity. However, BH did well in Thai patient services (36% of revenue), where revenue grew 10% YoY and 4% QoQ.
• (-) Normalizing EBITDA margin was at 36.1% in 4Q24, up from 35.6% in 4Q23 but down from 40.3% in 3Q24. The QoQ margin drop was due to rising SG&A expense on seasonality and lower revenue intensity.
• (+) Lower tax expense at Bt279mn, down 23% YoY and 34% QoQ, as BH had tax benefit from the BOI’s Investment Promotion for Community and Society Development.


Earnings forecast fine-tuned. We revise up our core earnings forecasts by 2% for each of 2025 and 2026, giving core earnings growth of 3% in 2025. The unexciting core earnings growth is based on a view of a slight drop in EBITDA margin to 38.9% (from 39.4% in 2024) as revenue intensity continues to normalize from 3Q24. In 2026, we expect a hiccup in BH’s core earnings growth from the opening of a new hospital in Phuket in 2H26. We maintain our Neutral rating with end-2025 DCF TP of Bt250/share (WACC at 7.5% and long-term growth at 3%).


Risks. Since international patient services contribute the majority of revenue, we are keeping an eye on the global economic slowdown and geopolitical risk that may cause clients to delay elective medical care and make it inconvenient for international patients to come to Thailand for treatment. We see ESG risk as patient safety (S): BH has adopted a variety of quality assurance systems to provide continuous patient care.
