4Q67: กำไรสุทธิต่ำกว่าคาด กำไรปกติเป็นไปตามคาด RJH รายงานกำไรสุทธิ 4Q67 ที่ 45 ลบ. ลดลง 54% YoY และ 79% QoQ ซึ่งต่ำกว่าที่ INVX ประเมินไว้ 27% จากผลกระทบเชิงลบที่สูงกว่าคาดจากการรับรู้รายได้ค่ารักษาพยาบาลโรคที่มีค่าใช้จ่ายสูง (AdjRW>2) จากสำนักงานประกันสังคมในอัตราที่ต่ำลง หากหักรายการนี้ออก พบว่ากำไรปกติอยู่ที่ 98 ลบ. ลดลง 1% YoY และ 54% QoQ เป็นไปตามที่เราคาดการณ์ สำหรับปี 2567 RJH รายงานกำไรสุทธิ 493 ลบ. เพิ่มขึ้น 17% YoY และหากหักรายการพิเศษออก พบว่ากำไรปกติอยู่ที่ 561 ลบ. เพิ่มขึ้น 34% YoY RJH ประกาศจ่ายเงินปันผล 0.2 บาท/หุ้น คิดเป็นอัตราผลตอบแทนจากเงินปันผล 1.3% XD วันที่ 10 มี.ค. 2568 และจ่ายเงินปันผลวันที่ 30 เม.ย. 2568

• สำนักงานประกันสังคมประกาศปรับลดอัตราการจ่ายค่ารักษาพยาบาลโรคที่มีค่าใช้จ่ายสูง (AdjRW>2) ลงมาอยู่ที่ 8,000 บาท/AdjRW (จากอัตราที่กำหนดไว้ก่อนหน้านี้ที่ 12,000 บาท/AdjRW) สำหรับบริการรักษาพยาบาลในช่วงเดือนก.ค.-ธ.ค. 2567 ส่งผลทำให้รายได้จากประกันสังคม (SC, 40% ของรายได้) ของ RJH อยู่ในระดับต่ำที่ 247 ลบ. ใน 4Q67 ลดลง 10% YoY และ 40% QoQ นอกจากนี้ RJH ยังบันทึกค่าใช้จ่าย SG&A อีก 56 ลบ. เพื่อสะท้อนการรับรู้รายได้ค่ารักษาพยาบาลโรคที่มีค่าใช้จ่ายสูง (AdjRW>2) ในอัตราที่ต่ำลงจากที่บันทึกไว้ก่อนหน้านี้ที่ 12,000 บาท/AdjRW ใน 3Q67 สำหรับบริการแบบชำระเงินเอง รายได้จากบริการ IPD (30% ของรายได้) เติบโต 11% YoY แต่ลดลง 7% QoQ และรายได้จากบริการ OPD (30% ของรายได้) เพิ่มขึ้น 2% YoY และทรงตัว QoQ
• RJH รายงานรายได้ภาษี 60 ลบ. เทียบกับค่าใช้จ่ายภาษี 25 ลบ. ใน 4Q66 เนื่องจากบริษัทได้รับสิทธิประโยชน์ทางภาษีจากการสนับสนุนเครื่องมือแพทย์ให้แก่สถานพยาบาลของรัฐ
คงประมาณการกำไร เราคงประมาณการกำไรปกติปี 2568 ของ RJH ไว้ที่ 530 ลบ. ลดลง 5% YoY เพราะถูกฉุดรั้งโดยการดำเนินงานเต็มปีแรกของโรงพยาบาลใหม่ โรงพยาบาลราชธานี หนองแค ในจังหวัดสระบุรี ที่เปิดให้บริการในเดือนต.ค. 2567 สำหรับการรักษาโรคที่มีค่าใช้จ่ายสูง (AdjRW>2) ในปี 2568 สำนักงานประกันสังคมได้ประกาศในจดหมายอย่างเป็นทางการว่าจะการันตีอัตราการจ่ายที่ 12,000 บาท/AdjRW มีผลบังคับใช้เป็นเวลา 6 เดือน เริ่มตั้งแต่วันที่ 1 ม.ค. 2568 (1H68) หลังจากนั้นจะมีการพิจารณาทบทวนอีกครั้ง และสำนักงานประกันสังคมระบุว่าจะจัดสรรงบประมาณเพิ่มเติมเพื่อคงอัตราการจ่ายไว้ที่ 12,000 บาท/AdjRW ตลอดทั้งปี 2568 ซึ่งเรามองว่าจะช่วยคุณภาพกำไรของ RJH เราคงคำแนะนำ NEUTRAL สำหรับ RJH โดยให้ราคาเป้าหมายสิ้นปี 2568 ที่ 24 บาท/หุ้น ซึ่งอิงกับ WACC ที่ 6.4% และการเติบโตระยะยาวที่ 2%
ปัจจัยเสี่ยง การปรับอัตราการเบิกจ่ายสำหรับโครงการประกันสังคม จำนวนผู้ป่วยที่เข้ามาใช้บริการชะลอตัวลง และภาระต้นทุนที่โรงพยาบาลใหม่ เรามองว่าปัจจัยเสี่ยงด้าน ESG คือ ความปลอดภัยของผู้ป่วย (S) ซึ่ง RJH ได้นำเอาระบบบริหารคุณภาพต่างๆ มาใช้สำหรับกระบวนการดูแลผู้ป่วยอย่างต่อเนื่อง
4Q24: Net profit misses, core in line
RJH reported 4Q24 net profit of Bt45mn, 27% below INVX estimate from a harsher effect than expected from lower payment for high-cost care (AdjRW>2) services by the Social Security Office. Excluding this, core profit was Bt98mn, down 1% YoY and 54% QoQ, in line with our estimate. We maintain Neutral on RJH with an end-2025 DCF TP of Bt24/share as we expect a short-term earnings hiccup in 2025 from the initial cost burden from the new facility.
4Q24: Net profit below estimates, core profit in line. RJH reported 4Q24 net profit of Bt45mn, plummeting 54% YoY and 79% QoQ, missing INVX estimate by 27% from a sharper impact than expected from the lower payment for high-cost care (AdjRW>2) services by the Social Security Office. Excluding this gives a core profit of Bt98mn, down 1% YoY and 54% QoQ, in line with our estimate. This brought 2024 net profit to Bt493mn, growth of 17%; excluding extra items, core profit was Bt561mn, up 34%. RJH announced a DPS of Bt0.2, 1.3% dividend yield. XD is March 10 with payment on April 30.
Highlights:
• The Social Security Office announced a lower rate for high-cost care (AdjRW>2) of Bt8,000/AdjRW (vs a predetermined rate of Bt12,000/AdjRW) for services in July–December 2024. This weakened RJH’s revenue from the social security scheme (SC, 40% of revenue) to Bt247mn in 4Q24, down 10% YoY and 40% QoQ. Additionally, RJH booked SG&A expenses of Bt56mn in order to reflect the lower payment for the service that had been earlier booked at Bt12,000/AdjRW in 3Q24. For self-pay services, IPD revenue (30% of revenue) grew 11% YoY but slipped 7% QoQ and OPD revenue (30% of revenue) increased 2% YoY and was flat QoQ.
• RJH reported tax income of Bt60mn vs. tax expense of Bt25mn in 4Q23 as it received tax benefit from donating medical equipment to public hospitals.
Earnings forecast maintained. We maintain our 2025 core earnings forecast of Bt530mn, a fall of 5%, dragged down by the first full year of operations at the new facility, Rajthanee Nongkhae Hospital, in Saraburi, opened in October 2024. For high-cost care (AdjRW>2) in 2025, the Social Security Office has announced in an official letter that it will guarantee a rate of Bt12,000/AdjRW, effective for six months from January 1, 2025 (1H25). A review will follow, and the Social Security Office says that it will secure an additional budget to maintain the rate of Bt12,000/AdjRW for the entire year of 2025. We expect this will help improve earnings quality. We maintain our Neutral rating on RJH with an end-2025 TP of Bt24/share, based on WACC at 6.4% and long-term growth at 2%.
Risks. Change in SC reimbursement, slower patient traffic and cost burden at new facilities. We see ESG risk as patient safety (S): RJH has adopted a variety of quality assurance systems to provide continuous patient care.
